This notice explains how this website handles appointment requests. It does not replace the practice’s separate notice of privacy practices for clinical care.

What the request form collects

The form asks for your name, phone number, email, whether you are a new or returning patient, preferred contact method and optional dentist preference. It also records your consent and a technical request identifier. Please do not send symptoms, medical history, insurance numbers or other clinical information through this form.

How the information is used

Payne & King uses the request to contact you about scheduling. The website uses Resend to transmit the request to the office by email. A copy is also sent to Key City Digital, the website provider. A successful form submission is not confirmation of an appointment or a guarantee of email delivery.

Website operation

The hosting and delivery services may process technical information needed to operate and protect the site, such as IP addresses and request logs. When enabled, Cloudflare Turnstile checks submissions for automated abuse. This website does not currently include advertising or analytics tracking. Clinical forms open in the separate Archy patient portal, which has its own privacy terms.
